One (1) Full-time, Monday – Friday, 8:30am – 5:00pm, 80 hrs b/w. Provides timely and accurate patient evaluation and needs assessment to achieve positive outcomes. Resolves specific service and system obstacles. Maintains effective relationships with external agencies, community resources and as needed, health plans. The population served is multi-cultural, and includes adults and pediatric patients, a predominantly elderly population, under-insured or non-insured patients and their families. There is also significant involvement with patients of all ages suffering the outcomes of substantial substance and physical abuse. Identifies patients/families who are in need of psychosocial, financial and/or community support to maintain an optimized level of health. Connects patients with community resources to ensure proper follow-up. Master’s degree in Social Work from a school accredited by the Council on Social Work Education (CSWE).
